After the request has been approved by the PTA, use the forms below for reimbursement or vendor checks. Items purchased by the PTA are to remain on campus at all times.
Reimbursement Request Form
Please use this form if you have already purchased an item and would like to be reimbursed. You can submit the form online and email your receipt to treasurer@canyonrimpta.org or you can download the form to fill out and turn in to the treasurer either by putting it in the PTA inbox or bringing it to the meeting. Please email treasurer@canyonrimpta.org if you've put a request in the PTA inbox.
Check Request Form
Please use this form if you have not yet purchased an item and would like the PTA to write a check directly to the vendor or purchase the item for you.
